If you are going to be selling/demonstrating/promoting your business in a different location other than your usual place of business during the NH Open Doors weekend, please type in the location information below (i.e. if you are partnering with other artisans to be together in one location, or you are an author signing books at a bookstore, or you will be demonstrating your art at a local library or retail store, etc.) Please include all information so the public can find you or contact the location with questions.

Choose the category that best represents what you will be featuring during the NH Open Door weekend. (If you feel you need additional categories, register again on a new form, select a new category, and pay an additional $65.) For each category, you must be the actual producer/craftsperson/owner who makes or creates the product or provides the service.
NOTE: Category selection will be approved by the NH Open Doors committee and the final list may be edited prior to being posted on the NH Open Doors web site.

Agreement:
I understand that participation in the NH Open Doors event is subject to approval by the two partnership organizations, New Hampshire Made and the League of New Hampshire Craftsmen.

By participating in the NH Open Doors event, I am agreeing to the following:

TO BE OPEN BOTH DAYS OF THE NH OPEN DOORS EVENT, November 7 - 8, during the hours I indicated on this form;

TO PROVIDE THE ACTIVITY/ATTRACTION/SALE indicated on this form on both days of the event;

TO PICK-UP MY PROMOTIONAL MATERIALS AT THE LOCATION SPECIFIED no later than one week prior to the event and post them in visible locations on both days of the event;

TO PROVIDE, IF POSSIBLE, A RECIPROCAL LINK TO THE WWW.NHOPENDOORS.COM SITE from my own business web site in order to help promote the event and generate additional traffic to my own site;

TO COMPLETE AND SUBMIT THE ON-LINE PARTICIPANT SURVEY no later than two weeks after the event;

TO ASSURE CUSTOMER SATISFACTION by greeting visitors warmly, providing information about my business, and responding courteously to all questions;

TO UNDERSTAND THAT MY $65 PARTICIPATION FEE IS NON-FUNDABLE once I have been approved and posted on the www.nhopendoors.com web site for any length of time.

Payment - Two Options:

1) Pay by credit card on-line:
If you choose to pay online, your credit card WILL NOT be charged until your application is approved by NH Made and/or the League of NH Craftsmen. Once you choose the "Pay Online" button below, please give it a few moments while it re-directs you to our on-line store checkout process. If you have never previously purchased from the NH Made Store before, you will be asked to enter your name, address, etc. in order for the checkout process to be completed.

2) Pay by check via mail:
If you choose to pay by check, a copy of this completed application will be sent to the e-mail you provided above with instructions for sending your payment to NH Made, PO Box 150, Andover, NH 03216.
