Agreement:
I understand that participation in the NH Open Doors event is subject to approval by the two partnership organizations, New Hampshire Made and the League of New Hampshire Craftsmen.
By participating in the NH Open Doors event, I am agreeing to the following:
TO BE OPEN BOTH DAYS OF THE NH OPEN DOORS EVENT, November 8 - 9, during the hours I indicated on this form;
TO PROVIDE THE ACTIVITY/ATTRACTION/SALE indicated on this form on both days of the event;
TO PICK-UP MY THREE DIRECTIONAL ROAD SIGNS AT THE LOCATION SPECIFIED no later than one week prior to the event and post them in visible locations on both days of the event;
TO PROVIDE, IF POSSIBLE, A RECIPROCAL LINK TO THE WWW.NHOPENDOORS.COM SITE from my own business web site in order to help promote the event and generate additional traffic to my own site;
TO DISTRIBUTE THE CONSUMER SURVEY to those who visit my location during the event and submit those forms to one of the event partners - NH Made or the League of NH Craftsmen - no later than two weeks after the event;
TO COMPLETE AND SUBMIT THE ON-LINE PARTICIPANT SURVEY no later than two weeks after the event;
TO ASSURE CUSTOMER SATISFACTION by greeting visitors warmly, providing information about my business, and responding courteously to all questions;
TO UNDERSTAND THAT MY $60 PARTICIPATION FEE IS NON-FUNDABLE once I have been approved and posted on the www.nhopendoors.com web site for any length of time.

2) Pay by check via mail:
